New Patient Registration Form
Provide your personal information and patient authorization. This populates your file before you arrive — no clipboard at the front desk.

Patient In-take Form & Symptoms
Describe the reasons for your visit and any current symptoms. Your TCM practitioner reviews this before the consultation so the initial diagnosis is focused.

Schedule Your Visit
Pick a date and time through the WUC TCM clinic online appointment system. First-visit slots are typically 45–60 minutes.

Appointment Day
Bring a photo ID and your health insurance card to the clinic. Your TCM practitioner will confirm your in-take, perform the initial diagnosis, and begin treatment.
What to bring on the day of your visit
Photo ID
Driver license, state ID, or passport. Used to verify the name on your in-take form.
Insurance Card
Health insurance ID card if you have one. The first-visit TCM diagnosis and acupuncture are free regardless.
Clinic hours: Tue–Fri 2:00 PM – 5:00 PM. Sat & Sun by appointment only.
Pay It Forward — Tree of Life
If you have Medi-Cal insurance, are uninsured, or are experiencing economic hardship, you do not need to pay for your Chinese herbal medicine.
