Patient In-take Form & Symptoms
Describe what brings you in today — current symptoms, any changes since your last visit, and treatment goals. Your practitioner reviews this before the session.

Schedule Your Visit
Pick a date and time through the WUC TCM clinic online appointment system. Follow-up visits typically run 30–45 minutes.

Appointment Day
Bring your photo ID and health insurance card. Your practitioner will review your in-take, adjust treatment based on progress since your last visit, and continue care.
What to bring
Same as your first visit: photo ID + insurance card. If anything has changed since last visit (new medications, recent diagnoses, new insurance), please mention it in the in-take form so your practitioner can update your file.
Clinic hours: Tue–Fri 2:00 PM – 5:00 PM. Sat & Sun by appointment only.
